Malcolm Fraser, MD

Community Medical Director

Malcolm Fraser, MD, CMD, is a Board Certified Geriatrician. He lived and practiced in LTC in Florida from 1980 to 2010 and then moved to Boulder, CO (to be closer to the Grandkids in Denver) and completed the his professional practice transition in 2012. In 1990 he founded Bay Geriatrics which provides Consultative Services in Post Acute Care. From 1996-1999 he was Corporate Medical Director, LTC Physicians, Nashville, TN. From 1999-2003 he was Florida Regional Medical Director of Health Essentials of Louisville, KY. He also served as a Corporate Medical Director for Beverly Enterprises from 1992-1996.

He served as President of Florida Medical Directors Association Chairman, Board of Directors of FMDA and Chairman of the FMDA Industry Advisory Board 1993-2010. He is a past scientific reviewer for the Omnicare Formulary and has twice served on the PharMerica P&T Committee and was Corporate Medical Advisor to NCS HealthCare.

In 2008-11 he was Principal/Co Investigators in several clinical trials. gov registered trials involving COPD and Chronic Idiopathic Constipation. He has presented multiple posters including “Surveying the Outcomes Of Lubiprostone” (STOOL) Pilot Study. This looked at the treatment of Chronic Idiopathic Constipation in a Geriatric population from both a clinical and regulatory perspective. The results of one study were published in JAMDA in June 2012 (Effect of cognitive Impairment on the use of the Diskus Inhaler).


Christopher Horton, MD

Medical Director – Rehabilitation Suites

Dr. Horton graduated from the University of Kansas School of Medicine and completed his residency in Family Medicine at the University of Colorado. Chris has always known that he wanted to provide care to the geriatric population, and feels that he can have a profound impact on the patient while seeing them in a skilled nursing environment.

He is Board Certified in Family medicine, and worked in private practice a short time before co-founding his company, Colorado Geriatric Care, PC in 2007. He started the practice with one vision; To provide better care. Currently, Colorado Geriatric Care, PC works with over 30 skilled nursing facilities and cares for 2,500 guests throughout the Denver metro area.

Recently, Chris has volunteered for Medical Wings International, proving medical care & expertise to the under-served in South America. You will often see Dr. Horton rounding in a facility followed closely by his beloved therapy dog, Mia.


Tom Lally, MD

Medical Director – Assisted Living

Tom Lally graduated from the University of Illinois at Chicago and planned to be a liver transplant specialist upon completion of his residency. Tom was awarded the chief residency in his final year, and was tasked to build a house call practice to help serve the veteran population in Chicago. When he moved to Denver, he knew he wanted to continue making house calls and treat his guests in the home, where he felt he could have the greatest impact.

Tom started his own practice, Physician Housecalls, LLC in 2003, and has been proving care to seniors in assisted living & independent living communities, as well as making visits to private residences ever since. Physician Housecalls, LLC currently has 16 providers and is providing care to over 2,200 guests all along the Denver metro area.

In addition to being the Chief Resident at the University of Illinois at Chicago, Dr Lally was recently awarded the prestigious Physician of the Year honor by the American Academy of Home Care Medicine. When Tom is not making patient visits, he enjoys spending his time with his wife and two daughters. Whether they are skiing or taking a hike, they love everything to do with being outdoors.
